EMPLOYMENT AT WILL: I acknowledge that my employment through StaffPoint is an “at-will” relationship that has no
specific duration. This means that I can resign my employment at any time, with or without reason or advance notice, and that StaffPoint has the right to terminate my employment
at any time, with or without reason or advance notice. I understand that accepting an assignment through StaffPoint in no way constitutes a guarantee of employment.
StaffPoint does not offer tenured or guaranteed employment.
A) I understand that I may be required to work various assignments throughout the course of my employment, and that the hours, days, salary and type of work may vary.
I understand and agree that as an employee, I will contact StaffPoint prior to the end of an assignment to request an additional assignment. I understand that if I fail to
contact StaffPoint within 24 hours of the completion of an assignment, this will be considered a voluntary resignation. I have also been informed that failure to contact
StaffPoint within 24 hours will have an adverse effect on unemployment benefits.
B) I understand that StaffPoint reserves the right to reduce my hourly wage to the current minimum wage rate (for the entire week) if I fail to give 48 hours working notice
that I will not be at my assignment.
C) I have provided StaffPoint with any limitations I have that would prevent me from accepting assignments, and will also inform them of any changes in my availability
or other status within the 48 hour requirement.
ATTENDANCE REQUIREMENT: I understand if I fail to show up for an assignment, or do not notify
the StaffPoint office, it will be considered a voluntary resignation and I will not be considered for future assignments. I agree to be in attendance and on time during all scheduled
assignments. Failure to be present and/or on time may result in my immediate termination and will have an adverse effect on unemployment benefits.
PRE-EMPLOYMENT DRUG, BACKGROUND AND CREDIT CHECK: I hereby authorize StaffPoint to conduct a drug screen and/or investigate my background
inclusive of criminal and credit records (if applicable to the position I am applying for) and verify this information. I, further authorize StaffPoint to provide the information
contained herein along with its findings, employment history, drug screen, and background results to our client, if necessary for assignment at their facility. I release StaffPoint and any
person affiliated with StaffPoint, from liability therefore.
REPAYMENT OF PRE-EMPLOYMENT COSTS: I agree to pay StaffPoint for the total cost of my pre-employment drug screen and background inclusive of
criminal and credit records (applicable to the position I am applying for) if I fail to pass the drug screen and/or background check or if I fail to complete 30 calendar days
of employment. This cost will be deducted from my payroll check issued by StaffPoint. The typical cost of a pre-employment drug screen is $30.00 (subject to change).
TEMPORARY EMPLOYMENT INFORMATION: I understand that should I accept an assignment, StaffPoint will be my
employer, and I will not be entitled to participate in any employee benefit plan, pension plan, 401 (k) plan, profit sharing, retirement, deferred compensation, welfare, insurance,
disability, bonus, vacation pay, severance pay or other similar plan, program or agreement offered by StaffPoint’s client to which I may be assigned. StaffPoint is a temporary
staffing agency and does not provide benefits to temporary staff members.
REPORTING INJURIES: I understand that should I accept an assignment and become employed by StaffPoint, I will
be expected to follow our policies and procedures should an injury occur on the job. StaffPoint policy requires that all injuries occurring on the job, regardless of how minor,
be reported to your immediate StaffPoint supervisor who will arrange proper medical care for you. If you are working away from the normal work site at the time of injury, our
policy requires you to report the injury to your on-site working supervisor AND to your StaffPoint representative. StaffPoint will then arrange necessary medical care and/or
transportation. If you are not at work at the time that you feel that you have the need to see a physician for a work related injury, please call StaffPoint at (913) 469-5800
and we will arrange all necessary medical care as well as transportation.
Any Staffpoint employee, who is injured on the job and medical attention is administered, will be subject to controlled substance screening. Failure to provide a sufficient sample for analysis, failure
by the individual to show up as directed for the analysis or a positive test result for any controlled substance will be grounds for immediate discharge and denial of any and/or all worker's compensation benefits.
StaffPoint has established a set of approved medical providers through OHS Compcare. Employees are NOT authorized to go to any other provider without written permission from
StaffPoint unless it is a medical (life threatening) emergency. Failure to go to a physician approved by the company may lead to the Employee incurring all the costs associated
with the medical provider and care rendered.
POLICIES AND PROCEDURES ACKNOWLEDGEMENT: I have been provided with a copy (via email and/or print copy) of
the StaffPoint policies and procedures, and understand that I do have the opportunity to ask any questions that I may have. I understand and accept the StaffPoint policies and
procedures and agree that failure to abide by them could result in immediate termination.
DECLARATION: I hereby declare that all statements contained in this application are true and correct and understand that
false or inaccurate information in the application will be basis for termination.
By submitting this online application I understand that this is not a promise of employment. If StaffPoint decides to hire me for either temporary or full time work, I will be
employed at will, and understand that StaffPoint reserves the right to terminate employment at any time for any reason. This means that either I or the Company can terminate
the employment relationship at will, at any time, with or without cause or prior notice. This at-will aspect of my employment cannot be changed, waived or modified, except by
an individualized written agreement to the contrary signed by me and the Company's President.
By completing this application I am certifying that the information contained is true to the best of my knowledge. Any misrepresentation on an employment application may result in termination of employment. Some employment with StaffPoint may require pre-employment background investigation and tests for illegal substances. I will be advised in advance if the job being offered requires drug testing.
I further understand that StaffPoint reserves the right to require me to submit to a test for the presence of drugs in my system prior to employment and at any time during my employment, to the extent permitted by law. I also understand that I may be required to take other tests such as personality or skills testing, prior to employment and during my employment. I understand that should I decline to sign this consent or decline to take any of the above tests, my application for employment may be rejected or my employment may be terminated.
I certify under penalty of perjury that all of the information contained in this application, any other documents filled out in connection with my employment, and any information
provided in any interview, is true, complete and correct, and understand that any misrepresentation, omission or falsification of this application or any information provided
by me may result in my disqualification from further consideration for employment or, if hired, dismissal from employment. I authorize StaffPoint to contact my former employers,
references, and any and all other persons and organizations for information bearing upon my qualifications for employment. I further authorize the listed employers, schools and
personal references to give the Company any and all information about my previous employment and education, along with any other pertinent information they may have.
This application will be considered current for a period of 180 days after it is filed. If I wish to be considered for employment after that period, I understand I must renew my application.
